Interview & Candidate Evaluation Forms
The University requires hiring officers and/or search committees to document the candidate evaluation, interview, and hiring decision making process (for example, interview questions, search committee member names, notes on applicant interview responses or application materials). This documentation may be created in any written format; however, it must be uploaded under the “interview notes” section of the hiring proposal submission.
Hiring officers may use the forms below to record their interview notes and questions as they evaluate and interview candidates.
