1095-C Forms Announcement

You do not need the 1095-C form to file or to attach to your taxes. However the Affordable Care Act (ACA) requires Penn to send these forms to certain benefits-eligible faculty and staff members.
The 1095-C forms were mailed on 2/16 from Equifax and are now available online. Go to the My Pay section of the secure U@Penn portal at www.upenn.edu/u@penn then select “My 1095-C form.”

The 1095-C form includes information about the health insurance coverage offered to you by Penn as well as information for each of your family members enrolled under your Penn benefits plan.
