Certification of Prior Employment

Apply for Prior Service Credit, which waives the one-year waiting period for employer contributions to the Basic and Matching Plans.

Employment Verification Form for Vesting

Have service at UPHS and other Penn institutions credited toward your vesting in the Basic and Matching Plans.
